We understand your desire to live independently at home and remain an active member of your community. Our aim is to empower you to maintain the lifestyle you desire.

What is Community Care?

Whether you need a little help around the home or require greater support with day-to-day activities, our Community Care program provides you with a variety of options for the assistance you would like to receive. As your needs change, we can adjust our services so you can continue to live the life you want.

A member of our dedicated and experienced team of nurses and carers will work in partnership with you, getting to know you and your family personally, in order to create an individualised care plan to assist you in achieving your goals. This is what makes Catholic Care of the Aged different.

What Services are Available?

Your care plan may include a combination of services including Home Care, Personal Care and Nursing Care. These services are available privately or as part of a Home Care Package.

Home Care

Services may include assistance with domestic responsibilities including cleaning, meal preparation and shopping.

Personal Care

Services may include assistance with showering and dressing as well as medication management.

Nursing Care

Services may include assistance with injections, catheter care and health monitoring.

How do I Access Community Care?

Private Services

To access community care privately, simply contact the Community Care Team on (02) 5525 3601.

Department of Veteran Affairs Nursing Care To access DVA nursing care, you will need to obtain a referral form which can be gained through your GP or by speaking to your Hospital Discharge Planner. Alternatively you can self-refer to the Community Care Team by calling (02) 5525 3601.

Home Care Packages

Accessing a Home Care Package is easy by following 5 simple steps.

1. Assess Eligibility

Contact My Aged Care to book an appointment with the local Aged Care Assessment Team. www.myagedcare.gov.au or 1800 200 422

2. Establish Needs

Make an appointment with the Catholic Care of the Aged Community Care Team on (02) 5525 3601.
 
3. Work out cost

The Australian Government subsidises a range of aged care services in Australia. If your personal circumstances allow, you may be asked to contribute towards the cost of your Home Care Package.

4. Accept Package

Your Community Care Team will determine a suitable, personalised package tailored to your individual needs.

5. Begin Service

Once you’ve accepted your Home Care Agreement and Plan, your care and services can begin.

Respite Care

As a Carer there may be times when other commitments take you away from your carer role or you may simply need a short break. Our Community Care Team can provide assistance to carers, family and friends, whether for a couple of hours or for a couple of weeks. Respite care is available privately or through your Home Care Package. Contact us for more information.

Extra Services

We believe that care should be different for each person and that sometimes there are occasions when you might need some extra options to enable you to do the things you want and live the life you desire.

As a Community Care client, you may also have access to Pastoral Care, Lifestyle Programs, transport and other services as needed. Contact the Community Care Team to find out what services you may be eligible for.
