Community Lifestyle Support
Funded by the Department of Health, this service provides a home-based outreach service to people living with a mental illness who are isolated in the community and find it difficult to access community resources. All referrals are made through the Mid North Coast Area Health Services Mental Health Case Management Team at Port Macquarie Base Hospital; this team is responsible for case management of all Community Lifestyle Support.
The program's objectives are to provide a range of relevant, responsive psychosocial rehabilitation interventions.
Assistance offered covers skill development or support in housekeeping, budgeting, shopping, using public transport, attending appointments and general living skills, as well as referral to appropriate generic and specialist services as needed. Clients are also encouraged to identify and use recreational and social facilities and opportunities. Community Lifestyle Support advocates on behalf of clients to improve access to resources and services. Ultimately, the aim of Community Lifestyle Support is to increase the skill level of consumers to the point where they are able to live independently with dignity and pride.
